Session Description: In this breakout session, we will explore how behavioral science is being harnessed to improve medication adherence, particularly for Social Risk Factor (SRF) populations, in alignment with the Health Equity Index (HEI).
Given the importance of health equity with pharmacy quality measures, this session will dive into how health plans can leverage data-driven insights to understand the specific barriers SRF populations face. Participants will gain insights into: How behavioral science, harmonized with health plan and 3rd party data scientifically identifies the barriers in order to then generate personalized interventions to engage SRF members and improve adherence at scale. Real-world case studies demonstrate the impact of these approaches on improving medication adherence rates and closing care gaps, resulting in better health outcomes and enhanced performance. By the end of this session, attendees will have an understanding of the possibilities in applying behavioral science to their efforts to promote medication adherence and health equity, driving measurable improvements in both member health outcomes and plan performance.
Learning Objectives:
Upon completion, participants will be able to describe strategies to identify and address social risk factors (SRF) that affect medication adherence.
Upon completion, participants will be able to design interventions that promote equitable care for diverse patient populations
Upon completion, participants will be able to compare data-driven interventions used to deal with SRFs and effective tools for promoting outcomes.
