Session Description: As CMS Star Ratings evolve to include Health Equity Index, it is paramount for health plans to consider how measures like medication adherence can be addressed differently in order to improve outcomes in the social risk factor (SRF) population. At Elevance, we have integrated screening and addressing social barriers within our pharmacist-led clinical programs to address health equity while improving medication adherence performance.
